Summary
This study is testing a new support program to help cancer patients, especially older or frail ones, safely transition from the hospital back home. It involves a nurse consultation and a system where patients report their symptoms and well-being using a smartphone or tablet after discharge. The main goal is to see if this program is practical to run and if patients find it helpful, with the hope of improving their recovery and reducing trips back to the emergency room.
