Summary

This study looks at whether telling adults with prediabetes about their personal diabetes risk helps them take action to prevent the disease. About 120 participants will either have a 30-minute talk with a healthcare team member, receive a simple message through their patient portal, or get usual care. The goal is to see if these approaches increase steps like joining a diabetes prevention program or starting medication.
