Care plans may boost Well-Being for breast cancer survivors

Summary

This study looks at whether a survivorship care plan (a personalized health roadmap) can improve quality of life and reduce stress for breast cancer survivors. About 100 women in early survivorship (up to 18 months after treatment) will take part. The goal is to see if having a care plan helps them follow healthy habits and feel more supported.
