Summary
This study is testing whether a personalized survivorship care plan, along with counseling sessions, can help breast cancer survivors manage their health and improve their quality of life after active treatment ends. It compares this approach to just receiving the care plan booklet with standard follow-up care. The goal is to see if the plan helps survivors access recommended follow-up care and reduces stress.
