Summary
This study will check if a wellness program at Bonton Farms helps people in the Bonton neighborhood improve their heart health and quality of life. About 300 adults will take part for 12 months, and researchers will track things like diet, exercise, sleep, and blood pressure. The goal is to see if linking medical care with social services can make a real difference.
