Summary
This study is testing if a new care model helps veterans with chronic kidney disease better control their high blood pressure. It compares patients who manage their own blood pressure medications with a pharmacist's guidance against those who just monitor at home and receive standard care. The goal is to see if this more active role for patients leads to better blood pressure control and long-term health.
