Summary
This study tested whether adding a specialized geriatric care program (called GRACE) to standard annual wellness doctor visits could improve care for older adults with multiple, complex health issues. Researchers worked with two primary care clinics to design and pilot this combined approach. The main goals were to see if it was practical to run this kind of program and if it could improve patient experience and reduce hospital stays.
