Summary
This study is testing whether a smartphone app that shows real-time blood sugar and activity data can help adolescents and young adults with type 2 diabetes better control their condition. For 90 days, participants will use the app along with a continuous glucose monitor and a Fitbit, receiving daily prompts to review their data and set goals. The goal is to see if this real-time feedback leads to improved blood sugar levels and healthier lifestyle habits.
