This trial tests whether placing a piece of amniotic membrane soaked in a stem-cell-derived solution (secretome) on the nerve during carpal tunnel release surgery can improve symptom relief and nerve function. Adults with moderate-to-severe carpal tunnel syndrome that hasn't improved with conservative care will be randomly assigned to standard surgery alone or surgery plus the membrane and secretome. Inclusion Criteria: * Adults aged 18 years or older. * Diagnosed with moderate-to-severe Carpal Tunnel Syndrome (CTS) based on electromyography (EMG) according to the Bland classification. * Persistent CTS symptoms that have not improved with conservative treatment. * Willing and able to provide written informed consent and comply with study procedures.
