Summary
This study is testing a digital tool called a 'Smart Family Doctor' that uses AI to help people manage their health after heart surgery or stents. It aims to see if the app helps patients better control their blood pressure, blood sugar, and cholesterol. The trial will involve about 950 adults who have had these procedures and are currently managing these conditions.
