Summary
This study is testing a smartphone app designed to help people who have had a stroke or mini-stroke (TIA) manage their health after leaving the hospital. The app connects to home blood pressure and glucose monitors and uses AI to give personalized advice. Researchers want to see if using this app for a year helps patients better control their risk factors and prevents another stroke or heart problem compared to standard care alone.
