Summary

This study is testing a support program to help Black patients with uncontrolled high blood pressure. It connects nurses, community health workers, and remote monitoring tools within 25 primary care clinics. The goal is to see if this team-based approach helps patients lower their blood pressure by addressing both medical and social needs.
