Heart teen transition study: nurses vs. doctors in adult care prep

Summary

This study looks at two ways to help teens with congenital heart disease (CHD) switch from pediatric to adult doctors. One group gets education from a nurse, the other from a doctor. The goal is to see which approach works better and to improve tools for finding adult heart specialists. About 450 people with CHD and their supporters will take part.
