Summary
This study looks at whether screening for social needs (like housing or food) and using an antiracist approach in primary care can help reduce chronic diseases such as high blood pressure, diabetes, depression, high cholesterol, and asthma. About 68,000 adults and children with these conditions will take part. The goal is to see if this approach can lessen racial health gaps.
