Summary
This pilot study tests a simple tool called 'Wake and Weigh' that helps older adults with heart failure track their weight daily. The goal is to see if this easy habit improves their quality of life and ability to manage their condition at home. About 42 participants will use the tool and complete surveys to measure its impact.
