Hospital at home: study tests early discharge with remote monitoring

Summary
This study looks at whether it's possible to send eligible patients home from the hospital early and monitor them remotely using a smartphone app and vital sign checks. The goal is to see if at least 30% of suitable patients can safely transition to this 'virtual ward' under current healthcare conditions. About 306 adults who are hospitalized and have a safe home environment will participate.
