Summary
This study tests a video-based telehealth program for stroke survivors. After leaving the hospital, participants get an iPad and a blood pressure monitor, plus telehealth visits with a nurse and social worker. The goal is to see if this program helps more people keep their blood pressure under control and reduces racial differences in outcomes. About 542 adults who had a stroke or mini-stroke and have high blood pressure are taking part.
