Summary

This study tests whether a personalized care plan, based on a patient's risk of future kidney problems and delivered through digital tools, can improve health after acute kidney injury. About 6,000 adults leaving Alberta hospitals will be randomly assigned to receive either the tailored plan or usual care. The goal is to reduce serious complications like kidney failure, heart attack, and stroke within two years.
