Summary

This pilot study tested whether combining health information technology with practice facilitation (coaching) helps clinics refer more people with diabetes to diabetes self-management education and services (DSMES). The study involved 22 clinics and measured referral and attendance rates. The goal was to see if these tools can improve how clinics connect patients to education that helps them manage their diabetes.
