New clinic model aims to end the 'Care Cliff' for adults with childhood disabilities

Summary
This study looks at a special clinic called the Transitional and Lifelong Care (TLC) program, designed for adults with conditions like cerebral palsy, spina bifida, or developmental disabilities that started in childhood. The goal is to see if this clinic helps patients and their caregivers get better, more coordinated care as they move from pediatric to adult health services. About 410 patient-caregiver pairs in Ontario will take part, and researchers will measure satisfaction and quality of life.
