Summary
This study tests a new way to deliver palliative care at home for 60 people with advanced heart failure or certain cancers. The care team grows as symptoms worsen, starting with a community health worker and adding a social worker, nurse, or doctor as needed. The goal is to see if this flexible approach helps match the right level of care to each person's changing needs.
