New study matches palliative care level to patient needs in real time

Summary

This study tests a new way to deliver palliative care that adjusts the care team based on how well a person is doing. People with advanced heart failure or certain cancers will receive a community health worker, and if symptoms get worse, a social worker, nurse, or doctor is added. The goal is to give the right level of support at the right time to improve comfort and quality of life. About 60 adults in Manhattan are taking part.
