Summary

This study tests whether a telerehabilitation coaching program can help people who have had an ischemic stroke feel more confident managing their health, lower risk factors like high blood pressure and cholesterol, and avoid going back to the hospital. Sixty participants will either receive the coaching plus standard care or standard care alone. The coaching includes phone calls, videos, and goal-setting over three months.
