Remote Check-Ins could keep kidney Patients' blood pressure in check

Summary
This study looks at whether using a home blood pressure monitor plus a dedicated case manager can better control blood pressure in people with chronic kidney disease who live far from clinics. About 146 participants in northern Alberta will either get usual care or extra help from a case manager who adjusts their medications. The goal is to see if this approach lowers blood pressure more effectively over one year.
