Summary

This study is testing whether home blood pressure monitoring, combined with support from a case manager, helps control high blood pressure better than monitoring alone for people with chronic kidney disease living in remote northern Alberta. Researchers will compare two groups over one year: one group uses home monitoring devices and gets regular guidance from a case manager, while the other group uses the same devices but follows up with their regular doctors. The goal is to see if this approach improves blood pressure control and slows kidney disease progression in areas with limited healthcare access.
