Summary
This study tests whether remotely mentoring local clinic teams can help them successfully start and run a comprehensive diabetes care program. The program includes telehealth support, community health workers, medication access help, and education. Researchers will compare health outcomes like blood sugar control and blood pressure over three years between 320 low-income Latino adults who receive this program and those who get usual clinic care.
