Summary
This study is testing a 4-week telehealth program called C-READY, designed to help teenagers with sickle cell disease learn the skills they need to manage their own health as they transition to adult care. The program involves one-on-one video sessions with a therapist to work on goals like managing medications and daily routines. Researchers want to see if this accessible, home-based training improves teens' readiness for self-care and their thinking skills.
