Summary

This study is testing if a video-based exercise and education program can help people with COPD after they leave the hospital. It aims to see if this 'telehealth' rehab can reduce the chance of being readmitted to the hospital and improve breathing, strength, and quality of life. The program involves live video exercise sessions three times a week for 12 weeks, compared to standard care alone.
