Summary
This study tests whether checking blood pressure at home and talking to a pharmacist by phone each week can help manage high blood pressure after an ER visit. Twenty-four adults will take part, with half using the telehealth program and half getting usual care. The program lasts 3 months and aims to see if this approach is practical and helpful.
