Summary
This study tests whether a telehealth program that includes a medication device and care coordination can help 300 adults with serious mental illness better manage their health at home. The program aims to reduce hospital and ER visits, improve medication adherence, and track conditions like diabetes and high blood pressure. Participants receive the Medherent device and ongoing support from their community mental health agency.
