Summary
This study works with stroke survivors, their carers, and healthcare staff to design and test a program that helps people stay physically active after completing a rehab exercise program. Many stroke survivors struggle to keep up activity on their own, raising their risk of another stroke. The goal is to create a practical support plan that fits their real-life needs.
