Summary
This study tested whether an intensive, team-based monitoring program could improve recovery and reduce costs for people who have had a stroke or heart attack. 859 participants received either this intensive follow-up care from nurses, doctors, and pharmacists or the usual standard of care. The main goal was to see if the more coordinated approach led to better long-term health and was cost-effective over two years.
