Summary
This study tests a 6-week WeChat-based program called SOCIAL_HF for elderly heart failure patients who feel socially frail. Participants join weekly video chats, do group tasks, and track daily health. The goal is to see if this online support improves social connections, self-care, and quality of life compared to standard education alone.
