Summary
This study is testing a new education and support program called 'CONNECTing to LungCare.' The goal is to help people who smoke have better conversations with their doctors about quitting smoking and getting screened for lung cancer. Participants will receive personalized information and follow-up support to encourage these discussions and connect them with resources. Researchers want to see if this approach is practical and helpful for patients and their healthcare teams.
