Summary
This pilot study looks at whether having a patient's doctor and pharmacist review their medications together is practical and helpful in Swiss primary care. About 250 adults aged 65 and older who take at least 5 regular medicines will take part. The goal is to see if this team approach can be used in a larger study to improve medication safety.
