Summary
This study tests a new tool designed to help doctors and older hospitalized patients have better conversations about end-of-life care. The goal is to make sure patients' treatment preferences are clearly understood, documented, and followed. Researchers will compare this tool to usual care in 1,200 patients aged 80 and older.
