Summary

This study tests a new tool designed to help doctors and elderly patients (80 and older) have better conversations about end-of-life care preferences. The goal is to ensure that treatments like life support match what patients truly want. Researchers will compare the tool to usual care in 1,200 hospitalized patients to see if it improves documentation and reduces unwanted intensive care.
