Can your phone keep you out of the hospital? new app aims to prevent medication mishaps

Summary
This study is testing a smartphone app called 'Smart About Meds' (SAM) to see if it helps people manage their medications better after leaving the hospital. The goal is to reduce the chance of patients needing to return to the emergency room or be readmitted to the hospital. About 3,250 patients will be randomly assigned to either use the app with their usual care or receive only their usual care after discharge.
