Summary
This study tests a mobile app called Smart About Meds (SAM) that helps patients manage their medications after leaving the hospital. About 3,250 adults who own a smartphone will either use the app or receive usual care. The app provides a medication list, side-effect checker, and pharmacist support to reduce ER visits, readmissions, and deaths within 90 days.
