Summary
This study tests a mobile app called Smart About Meds (SAM) that helps patients manage their medications after leaving the hospital. The app provides an updated medication list, pill images, and safety checks, with pharmacist support. Researchers will enroll 3,250 adults discharged from two hospitals in Montreal and track whether the app reduces emergency visits, readmissions, or death within 90 days.
