Tablets for treatment: can remote care manage blood clots?

Summary
This study is testing whether remote care using a tablet can help manage patients with low-risk blood clots (thromboembolic disease). 50 participants will receive standard treatment and a tablet to fill out questionnaires, learn about their medication, and have remote check-ins with doctors. The main goal is to see if this remote setup is practical and improves communication between patients, hospital doctors, and family doctors over 6 months to a year.
