First seen Apr 23, 2026 · Last updated May 24, 2026 · Updated 2 times

Summary

This study tests whether a smartphone app that tracks symptoms can help prevent complications after patients with multiple chronic conditions leave the hospital. About 1,300 adults will use the app to report how they feel, and doctors will be alerted if problems arise. The goal is to catch issues early and reduce hospital readmissions.
