Can balance training ease stubborn heel pain?
NCT ID NCT07754214
First seen Aug 10, 2026 · Last updated Aug 21, 2026 · Updated 3 times
Summary
This trial tests whether adding a supervised proprioception (balance) exercise program to standard stretching exercises improves pain, function, and disability in people with chronic plantar fasciitis, a common cause of heel pain. About 105 adults aged 18 to 65 with symptoms lasting at least 3 months will be assigned to either stretching alone or stretching plus proprioception exercises for 6 weeks. Researchers will measure pain intensity, pressure pain threshold, foot function, and biomechanical properties of the foot, ankle, and knee to see if the added exercises make a meaningful difference.
What this could mean
Our plain-language read of the trial. This is informational only, not medical advice or a prediction.
- Active substance
- A 6-week program of standard stretching exercises, with or without a supervised progressive proprioception exercise program.
- What this could lead to
- If adding proprioception exercises to standard stretching proves effective, it could offer a better non-drug way to reduce heel pain and improve foot function in people with chronic plantar fasciitis.
- What could go wrong
- This is a relatively small trial, and results may not apply to everyone. The added exercises require supervision and may not provide extra benefit beyond stretching alone.

Inclusion Criteria: * Age between 18 and 65 years. * Formal diagnosis of plantar fasciitis, characterized by sharp pain during the first steps in the morning, pain that increases with prolonged activity, localized tenderness on palpation of the medial calcaneal tubercle or proximal fascia, and a positive Windlass test. * Symptom duration of at least 3 months. * Heel pain intensity score of Visual Analog Scale (VAS) \> 3. * Voluntary agreement to participate, evidenced by signing the written informed consent form. Exclusion Criteria: * History of acute lower-extremity trauma. * History of prior foot or ankle surgery. * Corticosteroid injection to the affected heel within the preceding 6 months. * Presence of systemic inflammatory or rheumatologic disease. * Uncontrolled systemic disease (e.g., uncontrolled hypertension, uncontrolled diabetes mellitus). * Current or past history of malignancy. * Current pregnancy. * Body mass index (BMI) ≥ 30 kg/m². * Any musculoskeletal, neuromuscular, ocular, or vestibular disorder that could impair balance or limit the ability to stand unsupported. * Neurocognitive or psychiatric conditions precluding adherence to the exercise protocols or outcome assessments.
