Summary
This study tests whether a detailed review of medications can help older adults (65+) who take many drugs and have multiple health conditions. Researchers will compare readmission and emergency room rates between those who receive the review and those who do not. The goal is to improve communication between hospitals and general practitioners to reduce hospital returns.
