Summary

This study tests whether a detailed review of medications for older adults (65+) who take many drugs can lower their chances of returning to the hospital or visiting the emergency room. Researchers will compare patients who receive the medication review to those who do not. The goal is to improve communication between hospitals and general practitioners to better manage complex health needs.
