Summary
This study tests a personalized care model (TIME) for frail older adults receiving home care who are at risk of sudden health crises. About 150 participants will be followed for 6 months to see if the model helps them achieve their personal goals and reduces symptoms like confusion or agitation. The goal is to improve care and prevent unnecessary hospital or nursing home stays.
