Summary
This study tests a program called Pediatric Medication Therapy Management (pMTM) for children ages 2-18 who have complex chronic conditions and take 5 or more medications. The goal is to see if the program helps find and fix medication-related problems, like drug interactions or incorrect doses. About 371 children will take part, and parents will report on their child's symptoms.
