Summary
This study is testing whether adding trained community health workers to local pharmacies can help people get connected to social services. The goal is to see if this support leads to fewer trips to the emergency room or hospital. Researchers will enroll about 1600 adults at pharmacies in under-resourced areas of Western New York.
