Summary
This study is testing whether having community pharmacies screen people for social needs—like food or housing insecurity—and connect them to help can reduce trips to the hospital. It will involve about 1,600 adults at pharmacies in under-resourced areas of Western New York. The goal is to see if this approach improves health by addressing non-medical needs.
