Summary
This study tests whether a pharmacist-led program can help adult cancer outpatients take their medications correctly and avoid errors. One hundred participants will receive either usual care or extra pharmacist counseling, medication review, and follow-up. The goal is to improve adherence and reduce medication-related hospital visits.
