Pharmacists step in to prevent cancer medication mistakes

Summary
This study looks at whether a pharmacist-led program can help adult cancer outpatients take their medications correctly and avoid errors. One hundred participants will either receive usual care or extra pharmacist counseling and follow-up. The goal is to improve medication adherence and reduce hospital visits caused by medication problems.
