Summary
This study tests whether a pharmacist-led program — including medication review, counseling, and follow-up — can help adult cancer outpatients take their medications correctly and avoid errors. One hundred participants will be split into two groups: one receives standard care plus pharmacist support, the other gets standard care alone. The goal is to see if the pharmacist's help improves medication adherence and reduces mistakes.
